$10,000 Sign-On Bonus to eligible new employees joining UW Medicine Harborview Medical Center has an outstanding opportunity for a Registered Nurse (RN) to work in our Pioneer Square Downtown programs Field Based team which focuses on Harborview's mission population to address health equity issues by increasing access to care in our unique primary care settings, homeless shelters, housing programs or community settings for people without housing or vulnerably housed.

WORK SCHEDULE: • Part-time, 90% FTE, 36 hours per week • Four 9 hour shifts M-F

DEPARTMENT DESCRIPTION: The Pioneer Square Downtown programs focus on Harborview's mission population to address health equity issues by increasing access to care in our unique primary care settings, homeless shelters, housing programs or community settings for people without housing or vulnerably housed. This position will be sited in one of our Healthcare in Housing sites. Must be skilled at taking care of people with comorbid conditions including people with substance use disorders, mental illnesses, and complex physical health conditions with the ability to work collaboratively with outside agencies and assist with transitions of care from ED or hospital discharge.

PRIMARY JOB RESPONSIBILITIES: • Outreach and engage residents of the housing programs with the intention of engaging the residents in their healthcare • Record medical history, triage documentation, nursing visit and completion of orders • Provide health education, transitions of care and support chronic disease management in conjunction with primary care provider and community health worker • Support patients utilization of community and social resources • Provide direct patient care including: wound care, blood draws, point of care testing, vitals, immunizations, medication management, care coordination and nursing assessments

REQUIREMENTS: • Active, valid Washington State RN license required by start date. • Degree in Nursing from an accredited school of nursing • Two years of experience working as an RN. • Ability to work well with substance use disorders and mental illnesses • Ability to collaborate with outside agencies to facilitate primary care • Experience working with people who are homeless and economically disadvantaged • Experience working with diverse populations including limited English proficiency and immigrants
