University of Utah Care Coordinator in SALT LAKE CITY, Utah

Utahs pediatric mental health care access program, CALL-UP (Consultation Access Link Line to Utah Psychiatry), under a grant awarded by the U.S. Department of Health and Human Services, Health Resources and Services Administration (HRSA) Pediatric Mental Health Care Access Program (PMHCAP), is expanding to help more primary care providers identify, diagnosis, treat, and refer children/adolescents/young adults for behavioral health conditions.The CALL-UP Care Coordinator position in the Department of Psychiatry will be responsible for the continuum of care integration with the patient, primary care, and treatment protocols of psychiatric care. The Care Coordinator will serve as the liaison between primary care provider and the consulting psychiatrist. A critical component of the CALL-UP program, the Care Coordinator is expected to have extensive knowledge of regional resources and/or lived experience in order to successfully link primary care provider and families to the appropriate behavioral health services and supports based on their specific needs and insurance status. The Care Coordinator will also provide oversight of the workflow on the CALL-UP consultation line.
