Alvis House Case Manager in Lima, Ohio
Position Summary:
The purpose of this position is to manage cases for assigned clients and assure that each client receives continuous active treatment in accordance with his/her individual program plan (IPP), case plan, or equivalent. Ensure services are delivered in accordance with the mission and goals of the agency and correspond with the agency/governing body policies, procedures, and contracts. Employees will engage with clients on a consistent basis and utilize cognitive behavioral techniques to foster and promote healthy decision making and positive interpersonal skills.

Work Schedule: 8:00AM-4:00PM, Monday-Friday, off all holidays and weekends
Reports to: Program Manager/Facility Director
Essential Functions:
- Conduct and document intake procedures with assigned clients (e.g., review BOP or State ODRC regulations, agency policies, rules and regulations regarding employment, privileges, contraband, conduct)
- Work with assigned clients to develop and implements individual program plans, case plans, goals, and, meet with clients on a consistent basis to ensure compliance (e.g., contact employers regarding performance, attendance; review agency reports, logs regarding conduct, completion of assigned tasks, incidents reports, violations)
- Engage clients on a consistent basis utilizing behavior management programs and cognitive behavioral tools (e.g., role playing, mirroring, reflecting); facilitate structure activities in order to promote and foster the agency's pro-social culture
- Coach clients to utilize strategies to overcome identified barriers to success
- Document and record case activities and reports in applicable date management system(s); review case files to ensure on-going audit-ready status
- Maintain confidentiality of client information and files
- Act as an advocate for clients by identifying and coordinating with internal and external resources that provide needed programs and services (such as housing, employment, education, treatment services, healthcare)
- Ensure clients' needs and program requirements are met
- Ensure safety of clients and the community-at-large through reinforcing clients' positive pro-social behavior(s) and holding clients accountable for negative behaviors; document and report to supervising entities as appropriate
- Utilize effective problem-solving, prioritization, time-management, and organizational strategies
- Attend outside training conferences and/or seminars, in-service training sessions as required or assigned to increase professional knowledge and skills; apply new, relevant methodologies to meet client needs
- Administer emergency interventions, including CPR, First Aid, AED and Narcan (Naloxone) as needed
- Perform coverage and/or other related duties as needed
- May be required to transport clients and perform drug and alcohol testing (e.g., urine collection, breathalyzer)
Minimum Qualifications:
- Associates Degree in a related field
Required Experience:
- Minimum of one (1) year of experience in human services, corrections, or community-based services
- Work experience may be substituted for academic studies, exchanging one (1) year of work experience in a related field for one (1) year or academic education (total work experience needed in lieu of the combination of education and work experience is five (5) years
