Description

The Employment Case Manager works with clients in the CES division to assist them in finding employment in the community. This is done by working with county case managers, Department of Vocation Rehabilitation, local business, families and guardians of clients.

ESSENTIAL JOB FUNCTIONS AD RESPONSIBLITIES:

  • Contacts community based employers, as an employment consultant to secure paid employment and volunteer educational opportunities for referred individuals.

  • Researches and recommends community employment sites and activities, completes vocational assessments, conducts site evaluations and provides follow up as indicated.

  • Conducts career exploration and vocational opportunities by reaching out to employers and matching employer needs with individuals goals.

  • Assist Community Employment Services clients with activities such as: completing job applications and interviews, and providing training of work skills, work related Educational Information (Americans with Disabilities Act), appropriate behaviors and social interaction within the guidelines of Clarity Care’s positive proactive philosophy.

  • Provide on-site support to workers and employers.

  • Coordinate transportation for workers.

  • Case management services to include maintenance of progress notes, contact logs, and other required documentation.

  • Maintain accurate documentation of services according to policies and procedures.

  • Act as back-up job coach as needed.

  • Assist the Director of CES & QA with the marketing and development of the Community Employment Services Division of Clarity Care.

  • Other duties as assigned.

Qualifications

  • Bachelor degree in Human Services or a related field preferred.

  • Experience working with persons with disabilities.

  • Possess and maintain good physical, mental and emotional health and have capabilities to assist clients in any array of job sites.

  • Ability to work a flexible schedule according to the needs of the clients and employers.

  • Possess a valid driver’s license, submit proof of insurance, have dependable vehicle.

  • Able to pass WI Caregiver Background Checks including Department of Justice Criminal Record Checks, and/or that of previous states of residence.

  • Must be flexible and thorough, i.e., is able to cope well with the unexpected, meets deadlines, leaves no loose ends-attends to detail- all with minimum supervision.

  • Ability to work cooperatively with and under the direction and supervision of a variety of persons including agency supervisors, family members and guardians of clients, contract purchasers, etc.
