Great Lakes Management Co. RN Part Time 2 Days Per Week - Flexible Schedule in Champlin, Minnesota

Brand New Assisted Living and Memory Care community with 40 apartments. Job Objective(s) The role of the RN is to support the Director of Nursing by ensuring that the health and safety of all residents are met through on site visits with the residents and staff. Must be able to relate professionally with residents, staff and family members, display a positive work attitude and be able to work as a team with other staff members. Work in conjunction with the Director of Nursing to: *Assures residents maintain admission criteria, reassess and recommend alternative placement if necessary. *Assures residents have up to date service agreements per state regulations. *Monitors and maintains resident charts and clinical progress notes. *Communicates to physicians and other personnel (county caseworkers) changes in resident's needs or conditions. *Facilitates coordination of resident services with other service providers. *Assures all physicians orders are signed and implemented with-in 24 hours. *Communicates in resident's log any information that staff need to be aware of. *Performs supervisory visits 14 days after admission and at least every 90 days thereafter. *Works as a team player with all members of the residents care team. *Assures that staff clearly understands and follow all resident care procedures. *Monitors staff for accuracy of monthly resident blood pressures, weights and pulse as requested. *Shares resident and family concerns with pertinent staff if appropriate. *Is flexible and willing to expand and adapt to changes in the AL industry. *Assures compliance with regulations and is available for federal and state licensure surveys. Qualifications *Maintains a current state license as a Registered Nurse per state regulations. *A minimum of one year experience in home health, assisted living or long term care is preferred. *Good organizational skills with the ability to self-start and follow through with initiatives with a minimal amount of supervision. *Exhibits a caring heart with a passion to care for the elderly. *Possesses good judgment, problem-solving, and decision making skills. *Exceptional verbal and written communication skills. *Experience using a resident record computerized system *Experience working with seniors Environment Works indoors in a climate-controlled environment majority of the time.
